A DMO provides coverage to employees and eligible family members through a network of private practicing dentists. The employee and covered dependents must see the dental provider they have selected so that services will be covered. Please verify with customer service that the dentist is a participating provider when scheduling your appointment. There are no deductibles. Employee pays a copayment for the service provided according to a fee schedule. The plan provides free dental cleaning for children, ages 12 and under, and orthodontia coverage for children and adults.
Dental coverage is provided free of charge to the employee. Dependent coverage is optional and premiums are paid by the employee through payroll deduction.

The indemnity plan gives the employee freedom to choose his/her own dentist. It does, however, limit the employee's coverage based on years of participation in the plan. For enhanced benefit coverage, the employee and their covered dependents may elect to see a dentist listed as a network provider, see directory or call customer service direct.
Per calendar year - family limit is three times the individual deductible per class. Pre-existing conditions are covered for Class A and B benefits. Benefits will be based on reasonable and customary charges.
