Students
1. Student registers for class(es) - either credit or Continuing Education courses at Lone Star College-Montgomery.
2. Student submits Child Care Interest form and Child Care Enrollment Form to Student Services, Bldg. E, prior to dropping off their child(ren) at the LSC-Lone Star College–Montgomery Child Care Center.
3. The Child Care Enrollment Form includes the following:
- Registration and Enrollment Record Form
- Parent Handbook Acknowledgement Form
- Medical Information Record
- Teacher Information Sheet
- School Policy Form
- Notary Signature
- Current picture of your child
- Pictures of parent(s) - for safety purposes
All of these forms MUST be turned in prior to the first day of attendance at the Child Care Center.
Staff
1. Staff member submits Child Care Interest form to Student Services, Bldg. E, prior to dropping off their child(ren) at the LSC-Montgomery Child Care Center.
2. Staff member completes and submits Child Care Enrollment Record which includes the:
