EXAMPLE CHECKLIST Summer 2007 (LISTED BELOW)
The end of this semester has arrived. Grades are to be entered into My Records before 12:00 p.m. on Tuesday, June 16, 2007. (Remember to click on the “Submit” button. Then, (1) log out, (2) log back in to make sure your grades are recorded and will be downloaded into Colleague.)
The following materials must be turned in to April, June, or Barbara in F247 or C231.In order for your checkout process to run smoothly, please bring this checklist with you.

• 2 copies of My Records Grade Printout
- signed
• 1 copy of Grade Sheets with grading formula attached
• 1 copy of Class Roll Sheet
-with attendance
• Incomplete Contract Forms (applicable only for extenuating circumstances)
-signed by Instructor and Student with approval from chair if adjunct.
- must fill out grade change form; also submit completed form with signature without grade
• Outcome Report for Developmental Math
• Final Exams/Answer Keys
- For Math
• Any textbooks and/or CDs you will no longer be using
-if not returning, or not teaching the same course next semester
• Swipe Cards/ Swipe Keys
- anyone for music
-anyone not returning to teach next semester
• Parking Permit, if you are not teaching next semester
• Mailbox emptied?
